About The Position

Requirements

  • Minimum three (3) years of clinical experience required.
  • Minimum two (2) years of experience in case management required.
  • Minimum one (1) year of experience with the defined population required.
  • Masters degree in social work (MSW) required.
  • Licensed Clinical Social Worker - Certified (Maryland) within 6 months of hire.
  • Licensed Clinical Social Worker (Virginia) within 6 months of hire.
  • Independent Clinical Social Worker License (District of Columbia) within 6 months of hire.
  • Case Manager Certificate within 36 months of hire.

Nice To Haves

  • Experience with computer software programs in a Windows environment preferred.
  • Knowledge of community systems and resources in the defined service area preferred.
  • Knowledge of regulatory issues for the Mid-Atlantic area preferred.
  • Inpatient hospital social worker experience preferred.

Responsibilities

  • Collaborate with healthcare team members to coordinate appropriate services for members.
  • Conduct comprehensive psychosocial assessments to evaluate patient goals and resources.
  • Develop and monitor a plan of care in collaboration with the nurse case manager.
  • Identify and advocate for organizational and community resources to meet care plans.
  • Document case management activities in compliance with organizational standards.
  • Analyze patient and program outcomes to identify improvements in case management activities.
  • Facilitate applications for local, state, and federally funded programs.
  • Provide supportive counseling and education to members and their families.
  • Maintain knowledge of current case management standards and social work regulations.
