About the position

The Case Manager I/Social Worker provides essential services to chronically mentally ill patients, aiming to help them achieve their treatment goals and reduce the need for inpatient care. This role involves conducting psychosocial assessments, discharge planning, and community health referrals, while collaborating with healthcare professionals to ensure patient compliance with medical treatment. The position also includes delivering group and crisis interventions to address various mental health challenges, promoting overall mental and emotional well-being.

Responsibilities

  • Attend and participate in scheduled treatment team meetings.
  • Interview patients to obtain information concerning social history, treatment, and other pertinent information.
  • Manage the caseload of chronically mentally ill patients through ongoing assessment of individualized needs.
  • Execute therapeutic and/or crisis interventions, skill building, and case management for individuals.
  • Facilitate and document weekly individual and group interactions to aid patients in understanding their psychiatric treatment needs.
  • Maintain a cooperative relationship among healthcare teams by communicating information and participating in team problem-solving methods.
  • Fulfill clinical documentation requirements to evaluate the patient's progress in treatment and ongoing educational needs.
  • Maintain accurate records to ensure compliance with Federal and State regulations.
  • Refer clients to supportive services to supplement treatment and counseling.
  • Establish and maintain family contracts for each patient to ensure continuity of care and provide family education.
  • Coordinate the interdisciplinary assessment process and develop a psychosocial summary within 3 days of admission.
  • Coordinate discharge plans and continuation of care.
  • Stay informed of developments in theory and research.
  • Adhere to HIPAA guidelines.

Requirements

  • Bachelor's degree in social work from an accredited college or university.
  • Licensure to practice in the state of Texas preferred at the time of hire or must be obtained within 6 months of hire date.
  • Maintain current AHA Heartsaver CPR certification.
  • Physical ability to perform SAMA.

Nice-to-haves

  • Master's degree in social work from an accredited college or university.
  • Two years of health care experience or social services experience in a psychiatric setting related to counseling individuals in crisis/trauma situations.
  • Advanced training as a psychotherapist.
  • Bilingual.
