About the position

The Case Manager - Social Worker position at Ascension St. Vincent's Southside Hospital involves coordinating comprehensive care plans for patients from admission to discharge. This role emphasizes the integration of patient and family preferences into care plans, monitoring patient progress, and advocating for necessary resources to ensure effective healthcare delivery.

Responsibilities

• Coordinate the overall interdisciplinary plan of care for patients, from admission to discharge.
• Create care plans that integrate patient and family preferences and values.
• Monitor patient care through assessments, evaluations, and patient records.
• Advocate for resources and the removal of barriers to care.
• Maintain ongoing dialogue with supervisors and other health providers to ensure effective implementation of health plans.
• Act as a resource for adequate medical record documentation and appropriateness of services related to diagnoses and treatment options for post-discharge care.

Requirements

• High school diploma or GED with 2 years of cumulative experience, or an Associate's degree/Technical degree, or 4 years of applicable cumulative job-specific experience required.
• Clinical Counselor preferred.
• Social Worker specializing in Clinical Practice credentialed from the Florida Board of Clinical Social Work, Marriage and Family Therapy and Mental Health Counseling preferred.
