Responsibilities

• Manage the discharge planning process for residents in the short-term rehabilitation unit.
• Provide support to residents and their families during admission, stay, and discharge.
• Collaborate with interdisciplinary team members on daily admissions and discharges.
• Facilitate safe discharges into the community or other appropriate services.
• Arrange for skilled home care, therapy, and adaptive equipment as needed.
• Conduct care planning and MDS assessments.

Requirements

• Bachelor's degree in social work, human services, or a related field.
• Two years of experience in a healthcare setting.
• Experience with discharge planning and facilitating safe transitions for residents.
• Knowledge of care planning and MDS assessments.
