About the position

The SOR (State Opioid Response) Care Manager is responsible for providing pre-admission services to individuals struggling with substance use disorders. This role involves managing a caseload, assessing client needs, developing and implementing care management plans, and collaborating with peer recovery specialists. The Care Manager will engage clients referred from various entities and assist them in overcoming barriers to treatment, ensuring they receive the necessary support for recovery.

Responsibilities

• Develop and implement a Recovery Plan to help initiate and sustain an individual/family in recovery from substance use disorder.
• Maintain caseload and submit timely, detailed records.
• Utilize various techniques to engage clients and minimize failed appointments.
• Assist clients with applying to referral agencies/programs and for benefits, and with travel training.
• Accompany clients to court appearances and medical or other appointments.
• Assist clients with identifying and overcoming any barriers to treatment.
• Provide non-clinical crisis support and education.
• Transport participants to/from treatment or recovery support activities.
• Effectively utilize the agency's medical records software program(s) to complete required documentation.

Requirements

• Bachelor's degree in a relevant field such as Social Work, Psychology, Nursing, Rehabilitation, Education, Occupational Therapy, Counseling, Sociology, or a related human services field.
• Two years of experience providing direct services to individuals with Serious Mental Illness, developmental disabilities, or substance use disorders, or linking them to essential services.
• Valid driver's license and insurable status.
• Criminal background clearance.
