About the position

The Case Manager Specialist at Volunteers of America Los Angeles is responsible for providing intensive case management services to eligible clients. This role involves meeting with participants at least weekly and acting as the primary point of contact to coordinate services and care for assigned clients. The Case Manager handles a caseload of individuals in need of various human services resources and ensures that their needs are met through comprehensive assessments and individualized planning.

Responsibilities

• Conduct comprehensive screenings and assessments with individuals or families to collect relevant information for developing a case plan.
• Provide service linkages and support systems to ensure identified needs are met.
• Coordinate individualized planning with clients to address short- and long-term needs.
• Facilitate case conferences with residents and consult with the service team to monitor client progress on case plans.
• Advocate for clients before judicial, community, social service, and administrative bodies as needed.
• Facilitate support groups and encourage client participation as appropriate.
• Document and chart group education sessions clearly and concisely.

Requirements

• Associate's Degree in Social Work or related field or a combination of education and relevant experience (Two years' total).
• Minimum 1-year general experience in case management or similar social services role.
• Must be able to pass a fingerprint clearance, background check, including criminal history, personal references, employment and education verifications.
• Completion of VOALA Case Management training within 6 months of hire.
