About the position

The Restrictive Housing Case Manager Support Specialist at Graceville Correctional Facility is responsible for planning and implementing services for individuals in the restrictive housing unit. This role focuses on delivering evidence-based interventions and case management services to help residents achieve their goals and improve their behaviors, all while adhering to correctional standards and practices.

Responsibilities

  • Conduct individual sessions to deliver evidence-based interventions and case management services that target specific risk and need areas for change.
  • Work with residents to establish goals and objectives to be completed during their stay in restrictive housing and in the future.
  • Develop a treatment plan and coordinate services with other departments for areas outside the scope of practice, such as substance abuse or mental health services.
  • Assess residents' current criminogenic risk and need areas and coordinate services to address the highest needs area.
  • Develop and maintain a behavior management system within the unit that recognizes positive behaviors and gradually increases privileges based on behavioral progression.
  • Ensure proper documentation of assessment, individual and group intervention or case management sessions, and treatment plans in residents' files.
  • Audit files to determine that all services provided are documented.
  • Prepare progress reports on treatment plan goals and objectives and provide recommendations for continued support during re-entry to general population.
  • Promote the development of positive social skills through modeling appropriate behaviors and intervening when inappropriate behaviors are observed.
  • Maintain accountability of staff, residents, and property; adhere to safety practices.

Requirements

  • Bachelor's degree in social work, counseling, psychology, criminology or related field required.
  • Two (2) years of cognitive-behavioral intervention delivery preferred.
  • Valid driver's license with an acceptable driving record required, unless waived by management.
