• Provide transportation, case management, and coordination of services for patients.
• Monitor behavioral compliance issues and report concerns to the primary counselor or Program Manager.
• Assist patients in preparing for and obtaining employment and housing.
• Conduct case management groups and standby didactic group services.
• Provide orientation for new patients.
• Assist patients with community resources to obtain medication when necessary.
• Plan and coordinate sober leisure activities for all patients.

Requirements

• High School Diploma or GED required.
• Experience in substance use, detox programs, medication-assisted treatment, or behavioral healthcare is encouraged.
