About the position

The Case Manager Supervisor/Program Manager role at TriSource involves providing support to individuals with developmental disabilities. The position requires a strong leader who can coordinate services and supports for clients, ensuring their needs are met while maintaining compliance and achieving positive outcomes. The supervisor will manage a caseload of participants, lead a care team, and act as a liaison between clients and their families.

Responsibilities

• Be responsible for a caseload of 25-35 participants/staff to ensure needs are getting met, maintaining compliance, and help achieve positive outcomes for people served
• Lead, direct, and encourage care team members and clients on caseload
• Conduct assessments, person-centered plans, and create linkages for people to actively engage in the community
• Act as a liaison between clients, family/guardians, and other care team members
• Provide community-based services and advocacy
• Have management experience to lead, direct, inspire and hold the direct support team accountable to deliver high quality service and positive outcomes for the people served
• Demonstrate strong emotional intelligence and good communication skills

Requirements

• Strong leadership and managerial experience
• Experience working with the developmentally disabled population or several years working in the behavioral health field
• Bachelor's degree
• Knowledgeable about community resources and interventions to assist clients
• Ability to pass a background check
• Driver's license and personal vehicle
• Patience and compassion
