About the position

The Case Manager - Supports Planner at Total Care Services, Inc. is a full-time role focused on empowering individuals with disabilities to live independently in their homes and communities. The position involves identifying, accessing, and coordinating services and supports to maintain clients' health, safety, and independence, while also assisting with waiver eligibility determination and maintenance.

Responsibilities

  • Get to know clients and be an advocate for their needs.
  • Provide information about services and supports available in the community.
  • Help clients understand their options regarding care and support.
  • Assist clients in developing a personalized plan of service.
  • Support clients in realizing their goals and finding appropriate providers.
  • Make arrangements for the delivery of services and supports to clients.
  • Monitor client services to ensure they receive the necessary support.
  • Help resolve any issues that may arise during service delivery.

Requirements

  • Must be a licensed driver with reliable transportation and valid insurance.
  • Knowledge of resources and supports in the designated area.
  • Ability to work independently and in a community-based environment.

Nice-to-haves

  • Experience in case management or social work.
  • Familiarity with Medicaid and non-Medicaid home and community-based services (HCBS).
