About the position

The Transitional Age Youth (TAY) Case Manager is responsible for providing structured and therapeutic care to eligible youth and families under general supervision. This role involves direct service duties aimed at assisting youth in maintaining independence, managing crises, and providing behavioral interventions in collaboration with the TAY team. Responsibilities

  • Perform case management of clients including ethical evaluation, implementation of treatment plan goals, and documentation consistent with Medi-Cal required criteria.
  • Provide individual therapeutic and impromptu emergency response services and group interventions utilizing evidence-based models and best practices.
  • Deliver short-term and impromptu crisis interventions to service participants as necessary.
  • Provide client transportation when required.
  • Complete all required documentation in client files accurately and in a timely manner, maintaining updated client files.
  • Complete all required reports in a timely fashion while maintaining client confidentiality and adhering to HIPAA requirements.
  • Maintain flexibility to work in various settings such as clients' homes, schools, foster homes, and community settings at times convenient for clients.
  • Participate in the running of treatment groups and actively attend Group or Individual Supervision with the Clinical Supervisor and BWELL Practitioners.
  • Regularly attend and actively participate in BWELL Clinical Team Meetings.

Requirements

  • Bachelor's degree in a field related to mental health.
  • Minimum of one (1) year of full-time experience in a mental health setting focusing on psycho-social functioning, social adjustment, and/or vocational adjustment.
  • Ability to become QHMW certified within one (1) month of employment or be a licensed/waivered/registered mental health professional.
  • Strong computer skills.
  • Valid California Driver's License.
