About the position

Weston United Community Renewal Inc. is seeking a dedicated Case Manager for its Transitional Living Community in Harlem, New York. This full-time position involves working closely with clients who have histories of chronic homelessness, mental illness, and substance abuse. The Case Manager will be responsible for meeting with clients on a weekly basis to assist them in achieving their Individualized Living Plan (ILP) goals and connecting them with necessary community resources. This role requires a compassionate approach to help clients develop independent living skills and ensure compliance with all Department of Homeless Services (DHS) regulations. The Case Manager will work under the supervision of the Program Coordinator and will be tasked with acquiring necessary documentation for clients, facilitating access to community supports, and providing guidance in the development of independent living skills. The ideal candidate will have at least one year of experience in case management, particularly with adults facing severe and persistent mental illness, with or without a history of substance abuse. Strong interpersonal skills, excellent written and verbal communication abilities, and proficiency in Microsoft Office applications are essential for success in this role. Candidates should also possess experience in mental health, crisis intervention, and managing trauma-triggered behaviors. The position requires excellent time management skills, the ability to work under stress, flexibility, and a collaborative spirit. The Case Manager will also be expected to perform CPR, First Aid, AED, CPI, and Naloxone administration as needed. Responsibilities

  • Meet with clients weekly to assist them in achieving their Individualized Living Plan (ILP) goals.

Requirements

  • At least one year of experience in case management with a designated case load.
  • Experience working with adults who have severe and persistent mental illness, with or without a history of substance abuse.
  • Good interpersonal skills and excellent written and verbal communication skills.
  • Proficient in Microsoft Office applications including Word and Excel.
  • Knowledge of DHS CARES and AWARDS preferred.
  • Experience in mental health, crisis intervention, and managing trauma-triggered behavior.
  • Excellent time management skills and ability to work under stress.

Nice-to-haves
