Job Description: Case Manager (Tues-Sat) - Vagabond Inn Los Angeles, CA, USA Req #11148 Wednesday, August 7, 2024 Role: Case Manager (Tues-Sat) - Vagabond Inn Reports to: Program Director Department: Interim Housing Status: Full-Time, Non-Exempt, Non-Management Location: Vagabond Inn, 3101 S Figueroa St, Los Angeles, CA90007 Schedule: Tuesday-Saturday, 8:00am-4:30pm Benefits: Medical, Dental, Vision, Retirement Planning (403b),Employee Assistance Program (EAP), etc Job Summary: Through the use of evidence-based practices such as harm reduction,housing first, and trauma-informed care, the Case Manager supportsClients' housing stability, self-sufficiency, and communityintegration. The Case Manager partners with Clients who arecurrently homeless to assess and address barriers to housingplacement. Once Clients locate best-fit housing, The Case Managerworks to implement interventions with recently housed Clients tosupport housing retention and further coordinates multiple servicesto meet individualized needs. The Case Manager collaborates withClients to learn independent living skills and other life-skillsvital to successful transition from homelessness into permanenthousing. Essential Duties and Responsibilities: * Serve a caseload of approximately X clients, providing activecase management for all assigned cases, including meeting weeklywith each person individually * Act as the lead for client's housing-related case management,maintaining an active housing plan for each client on yourcaseload, based in creative solutions to homelessness * Maintain a current and thorough knowledge of community resourcesand utilize them to provide comprehensive, wrap-around services toclients, navigating them through the best options for their uniquesituations * Work collaboratively with clients to address barriers to housing,including linkages to income/benefits, mental health services,physical health services, etc. * Develop effective, trusting relationships with clients and use aclient-centered approach that includes motivational interviewing,harm reduction, and trauma-inf01med care * Advocate for your client's needs in the shelter and wheninterfacing with other agencies or service providers * Facilitate intakes for individuals who are referred into interimhousing * Create a supportive environment and ensure clients' progress,utilizing a strengths-based approach * Facilitate wellness, enrichment, and life skills groups * Provide crisis intervention when needed * Maintain client confidentiality at all times while followingagency, state and HIP AA regulations * Create and maintain accurate documentation of client information(intake, referrals, progress notes and service delivery) inHMIS/CHAMP database and in accordance with program funderrequirements * Participate in case conferences, individual supervision, andother staff meetings * Work in collaboration with other agency and program staff, inorder to coordinate services * Communicate effectively and in a timely manner with management,peers, and clients * Participate in evaluation and trainings in order to betterrespond to client/community needs * Other duties as assigned Qualifications: * Bachelor's degree in related field, strongly preferred but notrequired * Minimum of one ( 1) year of experience in case managementrequired * Experience and ability to demonstrate knowledge of issues facedby population served (at-risk, experiencing homelessness, and/orformally homeless ) * Knowledge of HMIS databases and the Coordinated Ent1y System,strongly preferred * Skilled in non-violent crisis intervention * Ability to self-motivate and be flexible in a fast pacedenvironment with minimal supervision * Strong interpersonal and team building skills * Strong knowledge of Microsoft Programs (Word, Excel, Outlook).*Computer skill test may be requested during the interview. Pleaserequest reasonable accommodation in advance, if necessary. * This position requires driving to multiple sites and transportingclients in personal vehicles at times. Candidates for this rolemust possess a valid drivers license in CA, valid insurance tocover the vehicle and use, an acceptable driving record for use ofvehicle while working for TPC, and a personal vehicle that is safeto use for such purposes. Work Environment: 1. Combination of field and office environment 2. Regularly required to sit, stand, bend; occasionally lift orcan-yup to 35 lbs. 3. On occasion walk or drive to different local sites 4. Will necessitate working in busy and loud environments 5. Will be exposed to elements like cold, heat, dust, noise andodor 6. My need to bend, stoop, twist, and sit throughout the day 7.
