About the position

The Case Manager position at Lutheran Social Services (LSS) focuses on supporting veterans facing homelessness through the Veterans Housing and Recovery Program. This full-time role involves conducting assessments, providing case management, and facilitating daily living skills classes to help veterans achieve independent living. The position emphasizes employment training and coordination of various social and mental health services.

Responsibilities

  • Conduct individual one-on-one meetings with program veterans.
  • Provide daily living skill classes.
  • Conduct assessments of customer needs through interviews and collateral contacts.
  • Develop casework plans with customers and perform necessary casework services.
  • Provide case management services and coordinate social, community, and mental health services.
  • Establish and maintain effective public and working relationships.
  • Maintain detailed case records and prepare statistical reports.
  • Make recommendations concerning case disposition and testify at hearings as required.
  • Participate in staff development programs and professional seminars.
  • Provide consultation to other social work staff and para-professionals as needed.
  • Serve as a representative of the agency to interested individuals and community groups.
  • Administer specialized programs within the community or agency as required.
  • Participate as a member of a quality team to enhance customer experience.

Requirements

  • Bachelor's degree in social work, sociology, psychology, or related field.
  • Two years of professional work experience in social services.
  • Comprehensive knowledge of counseling and casework principles and methods.
  • Considerable knowledge of welfare, health, mental health, and community resources.
  • Understanding of federal, state, and local regulations governing social services.
  • Ability to provide professional social work treatment to customers.
  • Strong organizational and planning skills.
  • Ability to establish and maintain effective public and working relationships.
  • Ability to prepare and present detailed reports.

Nice-to-haves

  • State Certified Social Worker (CSW) or Certified Professional Counselor (CPC) certification is preferred.
