About the position

The Case Manager (VSP) at WestCare Foundation plays a crucial role in the Prevention team, focusing on delivering quality services to veterans and their families under the Staff Sergeant Parker Gordon Fox Suicide Prevention Grant. This position involves case management, community resource linkage, and support for suicide prevention initiatives, ensuring veterans receive the necessary assistance and care.

Responsibilities

  • Support program alignment with the VA's National Strategy for Preventing Veteran Suicide (2018)
  • Conduct initial contact with eligible veterans and/or families for program eligibility
  • Schedule appointments to complete program eligibility process and review documentation
  • Develop and review recovery treatment plans and assessments
  • Complete service plans and provide information for continued peer support and family intervention
  • Facilitate weekly case management meetings with veterans and/or their families
  • Assist veterans and families in accessing community resources and services
  • Attend monthly integrated staff team meetings for care coordination
  • Maintain professional relationships with veteran organizations and community agencies
  • Maintain case files and input data in tracking systems
  • Submit weekly workplan/report on case management outcomes
  • Collaborate with Project Director and Evaluation team on data collection and reporting
  • Attend program conferences, meetings, and community events
  • Submit completed trainings for tracking purposes

Requirements

  • Previous military employment preferred
  • Valid driver's license required
  • Graduation from an accredited two-year or four-year college/university in a relevant field preferred
  • 3-5 years of experience in a veteran service-related field may substitute for degree
  • High School Diploma or equivalent required
  • Basic knowledge of veteran services, behavioral health, and social services
  • Experience with case management and administering assessments preferred
  • Excellent communication skills including public speaking and group facilitation
  • Experience with outreach and marketing
  • Ability to exercise good judgment and discretion
  • Ability to work well in a team environment
  • Adherence to ethical conduct and confidentiality
  • Culturally/linguistically sensitive to populations served
  • Research and evaluate data
  • Experience with data tracking systems
  • Ability to stay organized and manage time effectively
  • Proficient in Microsoft 365
