About the position

The Waiver Case Manager at Acumen Counseling Services is responsible for facilitating coordination and communication on behalf of individuals with intellectual and/or developmental disabilities. This role involves monitoring service delivery to ensure that the goals of the individuals are met effectively, maximizing their outcomes through collaboration with guardians and service providers.

Responsibilities

• Facilitates coordination, communication, and collaboration on behalf of the individual/guardian to achieve goals.
• Monitors the delivery of services to the individual at various service sites.

Requirements

• Bachelor's degree in Psychology, Sociology, Social Work, or other Human Services degree approved by DDID.
• Valid Kentucky RN license is acceptable in lieu of Bachelor's degree.
• 1 year experience working with individuals with intellectual and/or developmental disabilities.
• Must possess and maintain a valid driver's license and have access to reliable means of transportation for job-related use.
