About the position

The Case Manager at Star of Hope Mission is responsible for managing the life cycle of clients transitioning from homelessness to self-sufficiency. This role involves developing short and long-term objectives for residents, providing counseling based on Biblical principles, and ensuring compliance with program requirements. The Case Manager will maintain accurate records, conduct assessments, and assist residents with accessing social services, all while modeling Christian character and maintaining a supportive environment.

Responsibilities

  • Manage the life cycle of clients, including ministry and developing objectives.
  • Model Christian character and counsel residents using Biblical principles.
  • Conduct assessments through one-on-one interviews to identify appropriate programming.
  • Log pertinent information in the Case Worthy system.
  • Consult with residents weekly on their progress and document sessions.
  • Maintain accurate records on each resident.
  • Enroll residents in appropriate programs and required classes.
  • Assist residents with social services such as identification and food stamps.
  • Provide shadowing supervision to new Case Manager staff.
  • Participate in regular staffing meetings regarding residents' progress.
  • Refer residents to appropriate programs or counselors as needed.
  • Attend regular Case Management meetings and enforce rules and policies.
  • Screen residents for substance abuse and participate in assigned trainings.
  • Participate in ministry and prayer activities as appropriate.

Requirements

  • B.A. or B.S. in Social Science or related field, or equivalent experience.
  • 1 year of experience as a Case Manager, preferably with mental health issues.
  • Good computer skills, including knowledge of Microsoft Windows, Word, and Excel.
  • Valid Texas Driver's License with an acceptable driving record and current liability insurance.
  • Average to above aptitude in counseling and conflict resolution.
  • Ability to work respectfully and professionally in a Christ-like manner.
