About the position

The Case Manager for the Women's Residential Program at Catholic Charities, Diocese of Paterson, plays a crucial role in providing social service care to clients undergoing residential addiction treatment. This position involves managing a caseload of clients, coordinating with various agencies, and facilitating their access to necessary resources and services. The Case Manager is integral to the treatment team, ensuring that clients receive comprehensive support throughout their recovery journey.

Responsibilities

  • Provide case management for a caseload of residential addiction treatment clients, serving as a liaison between Straight & Narrow, Drug Court/Probation, and other social service agencies.
  • Ascertain client needs for essential documents such as birth certificates and social security cards, and assist in obtaining them.
  • Open and manage clients' welfare and Medicaid cases, ensuring all necessary documentation is submitted.
  • Notify clients' probation officers of their program participation and prepare required reports.
  • Schedule and conduct weekly meetings with clients, providing necessary follow-ups and support.
  • Coordinate Case Conferences with clients' probation officers to facilitate discharge planning.
  • Prepare monthly reports regarding each client for probation and other agencies.
  • Facilitate group sessions and didactics as scheduled by the supervisor.
  • Maintain accurate and up-to-date documentation in clients' charts according to DMHAS and CARF guidelines.
  • Attend MDT meetings to discuss clients' progress and needs.
  • Provide referrals to other agencies for continued care and support.
  • Encourage client participation in support groups such as AA/NA/GA.
  • Provide crisis intervention as needed.
  • Follow all Electronic Health Records (EHR) policies.

Requirements

  • Must have a High School Diploma or GED.
  • Must possess CADC or LCADC certification, or be actively pursuing one.
  • Computer literacy and typing skills are required.
  • Experience in discharge planning and knowledge of Section 8 housing is preferred.

Nice-to-haves

  • Licensed Clinical Social Worker (LCSW) or Licensed Associate Counselor (LAC) certification.
  • Experience in facilitating group therapy or didactics.
