About the position

The position involves providing advocacy, linkage, and referral services for individuals with chronic mental illness. The role includes case management, maintaining clinical records, and collaborating with community agencies to support high-risk populations. The candidate will be responsible for delivering Psychiatric Social Rehabilitation Services and ensuring compliance with agency policies and regulations.

Responsibilities

  • Provide advocacy, linkage, and referral services for chronically mentally ill individuals.
  • Conduct intake, screening, referral, and case management services.
  • Maintain an active liaison with community agencies serving high-risk populations.
  • Maintain the clinical records system in accordance with ODMHSAS, OHCA, CARF, and CREOKS regulations.
  • Attend and present cases in treatment team meetings regularly.
  • Read and be knowledgeable of all current CREOKS policies and procedures.
  • Maintain productivity averages of direct service to clientele as set by CREOKS for full-time employment.
  • Coordinate and manage the indigent medication/patient assistance program.
  • Submit billing and associated documentation as mandated by CREOKS policy.
  • Understand and follow all administrative, clinical, and programmatic policies and procedures of the agency.
  • Provide Psychiatric Social Rehabilitation Services as assigned by the Supervisor.
  • Maintain a full-time caseload as assigned by the Site Director and meet daily productivity averages.
  • Complete billing documentation daily and clinical documentation preferably by the next workday.
  • Interact with agency client families and other agencies to ensure appropriate therapeutic treatment.
  • Participate in mandatory trainings such as CAPE and CPR.
  • Implement rehabilitative curriculum and develop support materials based on best practices and evidence-based treatment models.
  • Teach rehabilitative skills necessary for daily living and successful community integration.
  • Treat all types of clients within the community based on staff competency levels.
  • Perform all other duties as assigned.

Requirements

  • High school diploma or equivalent.
  • 36 months of behavioral health experience.
  • 60 college credit hours in any field AND 12 months of behavioral health experience.
  • Bachelor's or Master's degree in any field with 6 months of behavioral health experience.
  • Maintain current case management certification.
  • Basic understanding of social and cultural factors influencing human behavior.
  • Knowledge of various treatment modalities.
  • Capability to work as part of a team.
  • Core competencies in relation to individuals with co-occurring disorders.
  • Case Management certification.
