About the position

The Service Coordinator at Rochester Spinal Association plays a crucial role in enhancing the quality of life for individuals living with spinal cord injuries. This position involves advocacy, assessment, and connection to essential services, ensuring clients receive the support they need to foster independence and navigate complex bureaucracies. The Service Coordinator will maintain a caseload of approximately 15 clients, primarily those with recent injuries, and will work collaboratively with various agencies to facilitate access to healthcare, housing, and other vital resources.

Responsibilities

  • Help clients connect to necessary health care services and therapies.
  • Assist clients in completing paperwork and scheduling appointments.
  • Make referrals to other agencies and services as needed.
  • Advocate for client needs and foster independence.
  • Facilitate client enrollment in programs such as job training and transportation.
  • Assist clients with applications for benefits and entitlement programs.
  • Maintain accurate records and reports of client interactions.
  • Build trust and rapport with clients by being available and accessible.
  • Research and develop familiarity with local social service agencies.
  • Communicate and collaborate with client's care teams.

Requirements

  • Bachelor's degree in social work or a similar field.
  • 3+ years of experience in service coordination, case/care management, or social work.
  • Excellent communication and interpersonal skills.
  • Strong organizational skills and attention to detail.
  • Proficiency with relevant community services for spinal cord injury support.
  • Valid driver's license, insurance, and personal vehicle.

Nice-to-haves

  • Personal or professional experience with spinal cord injury.
  • Awareness of health care and social services in the greater Rochester region.
  • Experience working with clients affected by chronic health conditions or poverty.
