About the position

The Case Manager role involves providing psychosocial support to individuals, families, or vulnerable populations facing chronic, acute, or terminal illnesses, as well as physical or developmental disabilities. The position focuses on patient education, counseling, and advocacy to coordinate necessary services and resolve crises. The Case Manager will also conduct clinical assessments, monitor client progress, and facilitate support groups to aid clients and their families in understanding and coping with their situations.

Responsibilities

  • Provide psychosocial support to clients and families coping with illnesses and disabilities.
  • Educate and counsel patients and caregivers on available resources and coping strategies.
  • Act as an advocate for clients to coordinate services and resolve crises.
  • Conduct individual and group counseling sessions to help clients overcome dependencies and adjust to life changes.
  • Identify environmental barriers to client progress through interviews and record reviews.
  • Monitor and evaluate client progress according to treatment and care plans.
  • Organize support groups and counsel family members to assist them in supporting the client.
  • Perform clinical assessments of behavioral health needs for clients seeking services.
  • Provide case management services related to identified treatment needs.
  • Train direct care staff in behavior modification techniques and treatment plans.
  • Refer clients or families to community resources for recovery support.

Requirements

  • Master's degree in a related field from an accredited college or university AND eligibility to be licensed.
  • Bachelor's degree in a related field from an accredited college or university AND two years of experience in social service delivery.
