About the position

The Case Manager at Sunstone Way is responsible for engaging with individuals experiencing houselessness, managing a caseload to meet their immediate needs, and helping them establish long-term goals towards sustainable housing. This role involves direct service, advocacy, and collaboration with various teams to ensure the well-being of participants while adhering to organizational policies and procedures.

Responsibilities

  • Conduct participant case management assessments and develop service plan goals (short term/long term).
  • Participate in case consultations and wrap-around meetings.
  • Facilitate client assistance needs, including information, referrals, and direct service.
  • Assist assigned caseload in finding sustainable housing.
  • Accompany participants to appointments as necessary.
  • Engage with individuals experiencing houselessness through maintaining an assigned caseload.
  • Research new resources for participants.
  • Perform gate shack duties, including letting participants and staff into the gate.
  • Create referrals for Peer Support Specialist as needed.
  • Respond to crises and write critical incident reports as necessary.
  • Fulfill duties of a Mandatory Reporter as outlined by the organization and relevant authorities.
  • Assist in the delivery of hygiene and essential supplies to individuals.
  • Log daily notes using cloud-based software, creating accurate records of services provided.
  • Utilize a strengths-based approach to support participants in communal living and interpersonal conflict resolution.
  • Communicate intentionally with participants and staff regarding shelter management issues.
  • Work effectively with village staff and participate in team building and organizational functions.
  • Coordinate and communicate information regarding participants with staff on the other side of the week.
  • Attend supervision, staff meetings, and required training sessions.
  • Distribute and record Tri-Met passes.
  • Support Case Coordinators and Navigation Specialists during unit checks.
  • Maintain effective communication with the Wrap-Around Team and other staff members.
  • Process paperwork and forms as required by the organization and relevant authorities.

Requirements

  • 1-2 years of experience in a case management role or similar work within a social services/non-profit agency.
  • Experience working with client information databases.
  • Knowledge of human behavior and performance, including individual differences in ability and personality.
  • Understanding of Housing First and Harm Reduction models.
  • Ability to interact effectively with diverse individuals and treat them with respect and dignity.

Nice-to-haves

  • Bachelor's degree in social work, psychology, or a related field.
  • Other social services certification or training.
  • Lived experience in relevant fields.
