About the position

The Case Manager/Mentor position at Thaise Educational & Exposure Tours, Inc. involves providing essential case management services to at-risk youth in a community-based or home environment. This part-time, contracted role focuses on linking clients with appropriate services, monitoring their progress, and serving as a liaison among clients, families, and social service agencies.

Responsibilities

• Linking and facilitating with appropriate services and resources identified in the service plan.
• Monitoring service delivery to evaluate client's progress.
• Documenting case management activities within the specified time-frame.
• Serving as a liaison for the client, family, referral source, and other social service agencies.

Requirements

• Graduation from an accredited university with a minimum of a Bachelor's Degree in Human Services or a related field.
• At least 1 year of experience working with at-risk youth.
• Knowledge of available resources for the specific population.
• Demonstrated proficiency in verbal and written communication skills.
• Ability to work with diverse groups and individuals.
• Maintains current annual training requirements.
• Must successfully complete all criminal background screens annually.
• Possess a valid driver's license and dependable vehicle for frequent travel.
• Knowledge of Microsoft Office and basic computer skills.
