About the position

The Case Manager at WestCare Foundation is responsible for providing comprehensive case management services to individuals in need. This role involves assessing service needs, developing care plans, and advocating for clients to ensure they receive necessary services. The Case Manager will collaborate with mental health clinicians and law enforcement to facilitate linkages and de-escalate situations in the field, embodying the mission and values of WestCare Foundation.

Responsibilities

  • Conduct assessments and periodic evaluations of individual needs for case management services.
  • Develop and periodically revise case management plans that include service activities.
  • Coordinate communication, referrals, and linkages related to client services.
  • Monitor service delivery to ensure clients have access to necessary services.
  • Advocate for patients and link them to physical and mental health care, transportation, and primary care retention.
  • Maintain client files in accordance with company policy and contract requirements.
  • Develop and maintain knowledge of available community resources and act as a liaison.
  • Maintain relationships with referring and supporting agencies and represent the agency at community meetings as requested.
  • Embrace and embody the mission, vision, guiding principles, clinical vision, and goals of WestCare Foundation.
  • Perform all other duties as assigned.

Requirements

  • Ability to forge effective relationships with law enforcement personnel.
  • Culturally and linguistically sensitive to populations served.
  • Knowledge of substance abuse and treatment is desired.
  • Excellent communication skills.
  • Ability to exercise good judgment and discretion.
  • Ability to work well in a team environment.
  • Adherence to the highest standard of ethical conduct, especially regarding confidentiality.
  • Professional appearance and demeanor.
  • Ability to obtain and maintain clearance from LEIE, MSIL, and EPLS.
  • Valid driver's license and ability to obtain and maintain WestCare's vehicle insurance.
  • Must be able to obtain and maintain Madera County Credentialing.
  • CPR Certification, First Aid Certification, and an annual Tuberculosis test.

Nice-to-haves

  • Two years of case management experience preferred.
  • BA degree in human services preferred; AA degree or two years of college credits in the human service field is required.
