About the position

The Case Manager at WestCare Foundation will collaborate with counselors to provide essential case management services to patients diagnosed with substance abuse disorders, mental illnesses, or both. The role involves assessing patient needs, educating them about community resources, managing electronic health records, and advocating for improved access to services. The Case Manager will also participate in clinical staffing decisions and contribute to the overall mission and goals of WestCare Foundation.

Responsibilities

  • Assess resource needs of patients.
  • Educate and refer patients to community agencies and businesses that address their needs for food, clothing, shelter, employment, transportation, etc.
  • Utilize and manage Electronic Health Record (EHR) systems for data entry and client information tracking.
  • Assist patients with establishing or re-establishing support systems within their community.
  • Act as an advocate on behalf of the patient to improve access to needed services.
  • Participate in clinical staffing decisions regarding patients' strengths and resource needs.
  • Complete documentation of services on provided forms in a timely manner.
  • Provide input into the patient's treatment needs and disposition.
  • Assist other WestCare staff in a collaborative manner to promote cooperation and professionalism.
  • Embrace and embody the mission, vision, guiding principles, clinical vision, and goals of WestCare Foundation.
  • Perform other duties as assigned.

Requirements

  • Bachelor's Degree in human services field required.
  • Ability to work collaboratively with others in a pleasant and professional manner.
  • Ability to work well in a team environment.
  • Ability to exercise good judgment and discretion.
  • Strong Microsoft Office Suite experience required.
  • Ability to complete work tasks within scheduled work hours.
  • Excellent verbal and written communication skills.
  • Willingness to seek out new work projects and contribute ideas for improvement.
