About the position

The Case Manager at Brockton Hospital is responsible for providing medical-surgical pediatric and obstetrical case management services to patients. This role ensures continuity of care for patients under specific physician groups and involves assessing, planning, coordinating, and implementing quality and cost-effective care across various settings including pre-admission, emergency, acute, and ambulatory care. The Case Manager actively identifies high-risk patients for timely interventions and collaborates with physicians and hospital staff to develop effective discharge plans while monitoring resource utilization.

Responsibilities

• Provide medical-surgical pediatric and obstetrical case management services to patients.
• Ensure continuity of care for patients under specific physician groups.
• Assess, plan, coordinate, and implement quality, cost-effective care for patients in various settings.
• Identify high-risk patients for timely case management interventions.
• Collaborate with attending and primary care physicians as well as hospital staff to develop discharge plans.
• Monitor appropriate use of hospital resources and facilitate service delivery.
• Conduct reviews of inpatient admissions using accepted criteria and guidelines.
• Serve as the primary liaison between physicians, patients, payers, and community providers.

Requirements

• High school diploma or equivalent required.
• Bachelor's Degree required; may be waived with experience commensurate with job expectations.
• Master's Degree preferred.
• Current license from Commonwealth of Massachusetts to practice as a Registered Nurse.
• Minimum of five years clinical experience in an acute care facility required.
• Prior experience in case management, utilization review, or discharge planning required.
