About the position

The Case Manager at Children's Rescue Fund is responsible for providing direct case management services to families in need, focusing on assessment, counseling, crisis intervention, referral, and advocacy. The role aims to ensure effective social service support plans are in place to facilitate the successful relocation of families to permanent housing. This position requires collaboration with various agencies and the ability to manage multiple cases effectively while maintaining compliance with state and city regulations.

Responsibilities

  • Complete family's intake assessment and service plan using the DHS Client Assistance and Re-housing Enterprise System (CARES).
  • Review accessible information from various agencies to support client needs.
  • Ensure timely updates of client information in the CARES system as required by DHS.
  • Provide advocacy and referral services to help clients access benefits and services for a smooth transition to permanent housing.
  • Coordinate services to stabilize family units and address issues such as substance abuse and parenting skills.
  • Assess client needs, formulate service plans, and document progress and significant interactions.
  • Develop and maintain helping relationships with families to support their residency and service goals.
  • Provide individual counseling and crisis intervention to stabilize families during displacement.
  • Maintain familiarity with program resources and refer families to appropriate services.
  • Facilitate groups to address family issues and enhance life skills for self-sufficiency.
  • Coordinate client services with other social services staff to ensure timely and appropriate service delivery.
  • Facilitate effective communication among staff and ensure smooth information transfer to administrative staff.
  • Perform other duties and special projects as assigned, including on-call responsibilities.

Requirements

  • Bachelor's degree in Social Work, Sociology, Psychology, Counseling, Criminal Justice, or Human Services preferred.
  • Associate degree in a related field considered with relevant experience.
  • Minimum of one year of experience in social services, counseling, crisis intervention, or housing services preferred for Bachelor's degree holders.
  • Minimum of two years of experience in social services for Associate degree holders.
  • Experience facilitating groups for homeless adults and knowledge of substance abuse issues is necessary.
  • Bilingual language ability is highly desirable.
  • Excellent written and verbal communication skills.
  • Strong interpersonal skills and proficiency in Microsoft Office.
