About the position

As a Case Manager with HELP USA, you will play a crucial role in assisting individuals experiencing homelessness to find stable housing and achieve self-sufficiency. This position involves identifying the underlying causes of homelessness, developing independent living plans in collaboration with a service team, and providing ongoing support and referrals to various services such as employment, education, and healthcare.

Responsibilities

• Help clients identify the causes of their homelessness.
• Coordinate with service team members to develop independent living plans.
• Meet with clients regularly to assist them in working towards their goals.
• Provide direction and referrals for employment, education, medical and mental health services, childcare, and substance abuse support.

Requirements

• Bachelor's degree in social work or a related field, or equivalent experience and skills.
• Ability to manage multiple priorities and complete tasks on time.
• Strong written and oral communication skills.
