About the position

The Case Manager at Agape Treatment Center is responsible for creating a safe and supportive environment for patients in the program. This full-time role involves monitoring patient activities, enforcing program rules, and providing case management services while maintaining confidentiality and compliance with policies.

Responsibilities

  • Monitors each patient throughout the daily schedule.
  • Enforces program rules and expectations and reports infractions to the Therapist and Clinical Director.
  • Monitors patient conformance with program schedule and reports issues.
  • Monitors patient participation and reports problems to supervisor.
  • Resolves conflicts through general and supportive advice.
  • Provides case management services.
  • Maintains the confidentiality of all patient and facility activities.
  • Complies with Policies and Procedures.
  • Accepts and utilizes supervision.
  • Addresses problems noted by supervisor.
  • Performs other assignments as requested by supervisor.
  • Demonstrates willingness to accept responsibility.
  • Completes all tasks with concern for work quality.
  • Being sensitive to cultural issues of a patient based on their job position.
  • Reports suicidality/homicide of a patient based on their job position.
  • Demonstrates competency and understanding of addiction based on their job position.

Requirements

  • A minimum of 1 year experience in substance abuse preferred.
