About the position

The Case Manager is responsible for evaluating the needs of low-income participants and developing tailored case management plans to assist them in overcoming barriers to self-sufficiency. This role involves coordinating various services and requires strong communication skills to effectively support clients in navigating available resources.

Responsibilities

• Evaluate the needs of low-income participants.
• Develop and implement appropriate case management plans.
• Coordinate services to assist participants in overcoming barriers to self-sufficiency.
• Provide information and referrals to clients regarding available programs and services.
• Maintain accurate records and documentation of case management activities.

Requirements

• Bachelor's degree in social work or a related field required.
• One (1) year experience in a human services related field.
• Social worker license preferred.

• Valid driver's license and satisfactory driving record preferred.
