About the position

The Case Manager will assist adults with disabilities and older individuals in assessing their needs and obtaining in-home and community-based services. The role aims to maintain clients' independence, divert them from institutional settings, and support their return home after hospital stays. The Case Manager will develop and administer service plans to ensure clients achieve the highest level of independent living possible while addressing acute situations.

Responsibilities

  • Perform comprehensive functional assessments and reassessments for clients.
  • Develop an appropriate plan of care to assist clients with identified outcome objectives.
  • Implement short-term and long-term intensive service plans, including authorization of specific in-home and community services.
  • Resolve client-directed concerns regarding service delivery quality.
  • Coordinate with providers of client services and maintain communication with them.
  • Plan for termination of services and follow up with clients.
  • Utilize in-house nursing staff for case consultation and service plan development.
  • Maintain manual and computerized client records.
  • Coordinate services with other community service providers, including mental health and disability service organizations.
  • Monitor the TXIX CM phone line, responding to and triaging situations as necessary.

Requirements

  • Master's degree in behavioral or health science and one year of paid social service experience, or
  • Bachelor's degree in behavioral or health sciences and two years of paid social service experience, or
  • Bachelor's degree and four years of paid social service experience, or
  • A relevant combination of education and experience may be considered.
  • Possession of a valid driver's license and reliable transportation.
