About the position

We are seeking a dedicated and compassionate Case Manager to join our team at BGC, Inc. in Lebanon, TN. The ideal candidate will play a crucial role in supporting individuals and families by providing guidance, resources, and advocacy. This position requires a strong commitment to improving the lives of clients through effective case management practices and collaboration with various service providers.

Responsibilities

  • Conduct comprehensive assessments to determine clients' needs and develop individualized service plans.
  • Provide ongoing support and case management services to clients, ensuring they have access to necessary resources.
  • Facilitate communication between clients and service providers, including social services, healthcare, and educational institutions.
  • Monitor client progress and adjust service plans as needed to ensure goals are met.
  • Implement crisis intervention strategies when necessary to support clients in distress.
  • Maintain accurate documentation of client interactions, progress notes, and case files in compliance with organizational policies.
  • Advocate for clients' rights and needs within the community and facilitate access to relevant programs such as Section 8 housing assistance.
  • Collaborate with multidisciplinary teams to enhance service delivery and outcomes for clients.

Requirements

  • Strong background in social work or related fields with experience in child welfare or behavioral health preferred.
  • Proficiency in motivational interviewing techniques to engage clients effectively.
  • Experience in mentoring individuals or groups, particularly in workforce development settings.
  • Ability to supervise and guide support staff or interns when necessary.
  • Knowledge of crisis intervention strategies to assist clients during challenging situations.
  • Familiarity with childcare practices and resources available for families in need.
  • Excellent communication skills, both written and verbal, with the ability to build rapport with diverse populations.
  • Strong organizational skills with attention to detail for maintaining accurate records.
