About the position

The Case Manager position at Beacon Health System is a full-time role focused on assessing and addressing the post-hospital needs of patients. The Case Manager collaborates with patients, families, and healthcare teams to ensure a smooth transition from hospital to home or other care settings, particularly for medically complex patients. This role requires strong communication skills, clinical expertise, and the ability to navigate community resources effectively.

Responsibilities

• Meet with patients and families to assess post-hospital needs and facilitate linkage with community services.
• Ensure a well-planned process from admission to discharge for medically complex patients.
• Collaborate with the interdisciplinary team to assess clinical readiness for transfer and discharge.
• Document patient assessments promptly and completely.
• Develop appropriate discharge plans in conjunction with other disciplines.
• Investigate community resources and present recommendations to patients and families.
• Advocate for patients with payers to obtain coverage for needed services.
• Provide education and facilitate linkages with community services and resources.
• Participate in discharge planning meetings and assist in scheduling conferences with patients and providers.

Requirements

• Valid RN License in Indiana
• Valid CPR Certification
• Master's degree in Social Work or a related field
• Minimum of three to five years of job-related experience
• Strong knowledge of medical and clinical processes
• Outstanding interpersonal skills with a focus on listening and conflict resolution.

Nice-to-haves

• Experience in case management or social work
• Knowledge of reimbursement processes and community resources
• Certification in a specialty area related to nursing or social work.
