About The Position

The Case Manager at HealthCore Clinic is responsible for assessing patients' needs and collaborating with counselors and other care providers to design effective treatment plans. This role involves follow-up and documentation of patient progress, as well as providing behavioral health education to individuals and groups. The Case Manager plays a crucial role in coordinating care and ensuring that patients receive the necessary resources for their mental health needs.

Requirements

• Bachelor's degree in social work, psychology, or counseling.
• 3 to 5 years of previous experience as a case manager or equivalent combination of education and experience.
• LBSW or LMSW with LAC required; LCAC preferred.
• Previous experience with Electronic Medical Records (EMR) preferred.
• Excellent written and verbal communication skills.
• Strong organizational skills and attention to detail.
• Ability to work effectively with vulnerable populations.

Nice To Haves

• Experience in behavioral health education and outreach.
• Familiarity with community health services and resources.

Responsibilities

• Assess patients' needs and work with care providers to design treatment plans.
• Document patient progress and maintain comprehensive reports as required.
• Coordinate care for patients, including specialty care and follow-up on test results.
• Develop and implement case management plans tailored to individual client needs.
• Provide self-management support and tools to patients to promote healthy behavior changes.
• Conduct public education outreach activities to raise awareness about services.
• Communicate effectively with team members to ensure patient engagement and follow-up.
• Participate in quality improvement initiatives and maintain partnerships with community organizations.
