About the position

The Case Manager at StepStone Family & Youth Services is responsible for ensuring quality care for clients by assessing their needs and coordinating appropriate services. This role involves direct communication with referral sources, overseeing pre-screening assessments, and maintaining relationships with various stakeholders to facilitate client care and satisfaction.

Responsibilities

  • Receives/responds to incoming calls from referral sources/potential clients to identify their needs.
  • Completes viable referrals by conducting pre-screening assessments and preparing necessary paperwork.
  • Oversees and supervises field staff involved in the pre-screening process.
  • Accesses and manages national/state/company account information relevant to client services.
  • Consults with third party representatives regarding client benefits and reimbursement procedures.
  • Contacts referral sources to inform them of case acceptance and the clinical team assigned.
  • Develops and maintains knowledge of company services and community resources.
  • Maintains relationships with referral sources and tracks satisfaction levels.
  • Implements operational processes to ensure compliance with company policies and regulations.
  • Participates in quarterly growth planning meetings and special projects.

Requirements

  • Bachelor's degree in a human services field or nursing field.
  • Valid driver's license.
  • Two years of experience in case management or a related discipline.
  • Strong verbal and written communication skills.
