About the position

The Case Manager is responsible for conducting functional assessments and coordinating community-based services for functionally impaired older adults. This role emphasizes client-centered principles to promote self-sufficiency and ensure a continuum of care. The Case Manager will engage in client interactions through home visits and telephone calls, review care plans, and participate in community outreach to enhance the Senior Services program.

Responsibilities

  • Conduct functional assessments for older persons to determine their needs.
  • Coordinate appropriate community-based services for functionally impaired clients.
  • Utilize client-centered principles to facilitate service coordination.
  • Conduct telephone calls and home visits to assess client safety and service adequacy.
  • Review care plans and perform semi-annual/annual reviews through home visits.
  • Participate in staff meetings and community outreach activities.
  • Ensure timely completion of APS referrals and follow-up activities.
  • Conduct peer-reviews as directed and maintain updated client case records.
  • Complete ongoing training requirements as directed.
  • Develop informal support networks for clients and caregivers as needed.
  • Perform emergency call-downs to clients during disasters.
  • Maintain confidentiality and professionalism in all interactions.

Requirements

  • Bachelor's Degree in Human Services or a related field.
  • One year of Case Management experience required.
  • Valid Florida Driver's License and insurable by the company's insurance carrier.
  • Knowledge of available community resources and state/federal regulations.
