About the position

The Case Manager position at Grand Central Neighborhood Social Services Corporation involves providing comprehensive support to single homeless adults through case assessments, service planning, and coordination of services. The role requires facilitating group sessions, maintaining accurate client records, and conducting outreach to potential clients. The Case Manager will work closely with clients to develop individualized service plans and ensure they receive the necessary resources and support.

Responsibilities

  • Develop and complete case assessments, service plans, and housing contracts with each client in care.
  • Provide referrals to and coordinate services with adjunct services as required, including assisting with employment and training/placement, school enrollment, and health care services.
  • Facilitate two groups a week related to the needs of the homeless population.
  • Prepare progress notes and bio-psychosocial histories as per agency standards.
  • Complete housing packages, self-housing contracts, and compliance contracts as required.
  • Maintain accurate charts as per agency standards.
  • Complete daily, weekly, and monthly reports and assessments of clients utilizing FileMaker Pro, CARES, and Excel databases.
  • Meet regularly with the immediate supervisor to enhance professional growth and review case management services.
  • Assist with completing intake assessments as needed.
  • Conduct in-house outreach of potential clients informing them of the services provided.
  • Participate in Social Services fairs and other events.

Requirements

  • Bachelor's degree in a relevant field.
  • Minimum of 2 years experience working in the field of Social Services with dual diagnosed clients and the homeless population.
  • Proficient in Microsoft Office Applications (Word, Outlook, Excel).
  • Bilingual in Spanish and English preferred.
  • Strong organizational and planning skills, work management and prioritizing skills.
  • Strong verbal and written communication skills.
  • Problem-solving ability and attention to detail.
  • Ability to work as part of a team.
