About the position

The Case Manager/Advocate at Youth Advocate Programs, Inc. is responsible for providing direct case management and advocacy services to clients and their families as part of the Merrimack County Intensive Service Option (ISO) In-Home program. This role emphasizes a strength-based approach using the wrap-around model, maintaining a manageable caseload, and ensuring compliance with documentation standards.

Responsibilities

  • Initiate, organize, plan, develop, and implement direct case management and advocacy services to assigned clients and their families.
  • Maintain a caseload of no more than 10 families per month on average.
  • Provide a minimum number of face-to-face hours with families each week.
  • Complete documentation in accordance with New Hampshire Medicaid standards.
  • Serve as a positive role model for youth in community, school, and home settings.

Requirements

  • Bachelor's degree in social work, psychology, education, or a related field with an emphasis in human services.
  • Two years of experience working with children and families.
  • Demonstrated communication skills, both oral and written.
  • Experience in the provision of home and community-based services.
  • Reliable transportation, valid driver's license, and current automobile insurance coverage.
