The Case Manager at Lutheran Social Service of Minnesota is responsible for providing case management services to youth involved in truancy prevention programming. The position emphasizes providing wrap-around services to meet the individual needs of program participants, ensuring they receive the necessary resources and support for their personal development.

Responsibilities

• Provide case management services to youth in truancy prevention programming.
• Coordinate with county social workers, foster parents, parents, probation, and community agencies.
• Develop and implement case management plans tailored to individual youth needs.
• Provide wrap-around services to address the goals set with the youth.
• Complete intakes and set/track goals for program participants.
• Advocate for youth and submit program reports/data for management.

Requirements

• Bachelor's degree in Social Service or related field.
• One year of experience working with youth, including life skills training and group facilitation.
• Valid driver's license and acceptable driving record.
• Reliable transportation and valid insurance.
• Strong organizational skills.
