About the position

The Case Manager at Beth Israel Lahey Health Behavioral Services plays a crucial role in supporting individuals with substance use histories through a collaborative and patient-centered approach. This per diem position involves working within a 40-bed detoxification program, providing intensive treatment and guidance for clients transitioning to long-term care. The Case Manager is responsible for conducting assessments, formulating treatment plans, and offering counseling services to help clients achieve their treatment goals.

Responsibilities

• Participates in assessments and aids in formulating effective treatment plans.
• Conducts discharge planning and assists in substance use counseling and case management services.
• Assesses patient's continuing care needs and arranges for transfer or admission to appropriate levels of care.
• Liaises with self-help organizations and appropriate after care resources.
• Attends staff meetings and participates in client evaluations, case conferences, and group programming activities.
• Counsels clients individually and in family or group sessions to assist in achieving treatment goals.
• Monitors client conditions to evaluate the success of therapy and adapts treatment as needed.
• Refers clients to other support services such as medical evaluation, social services, and employment services.
• Prepares and maintains reports and case histories.

Requirements

• High school diploma or GED required.
• Associates or Bachelor's Degree in Human Services or related field preferred.
• A minimum of one year related experience and/or training in the area of addiction treatment.
• Valid Driver's License in good standing preferred.
