About the position

The Case Manager position at Lutheran Social Service MN involves providing essential case management services to youth under the age of 25 who have experienced sexual exploitation or trafficking. The role focuses on managing client intake and discharge processes, developing case plans, and facilitating regular meetings with program participants. The Case Manager will also work collaboratively with families, professionals, and community organizations to ensure comprehensive support and successful transitions for clients.

Responsibilities

• Provide case management services to individuals and youth in transitional living and/or within the community.
• Manage the client intake/discharge process.
• Develop case plans and goals with program participants.
• Monitor and assist clients in carrying out case plan tasks.
• Facilitate weekly client groups and/or recreation activities.
• Build and maintain collaborative relationships with community organizations and employers.
• Complete assessments and determine eligibility for government and community services.
• Make referrals and develop a comprehensive follow-up system for services.
• Maintain accurate and current case files, program records, and reports.

Requirements

• Four or more years of a combination of education and experience in working with similar clientele.
• A valid driver's license and acceptable driving record.
• Computer literate with strong written and oral communication skills.
• Strong organizational skills and problem-solving abilities.
• Ability to demonstrate professional boundaries and promote a positive, supportive atmosphere.
• Knowledge regarding community programs and referrals.
