About the position

The Case Manager position at Volunteer Behavioral Health involves providing comprehensive case management services to adults and children, focusing on their strengths and needs. The role includes assisting individuals in accessing medical, social, educational, and other support services, ensuring they maximize the benefits available to them. Case managers will work both in the office and within the community, performing activities such as care management, coordination, and health promotion.

Responsibilities

• Initiate, complete, update, and monitor the progress of comprehensive person-centered care plans.
• Participate in the patient's physical health treatment plan as developed by their primary care provider.
• Support scheduling and reduce barriers to adherence for medical and behavioral health appointments.
• Identify and facilitate access to community supports such as food, shelter, clothing, and legal resources.
• Provide high-touch in-person support to ensure treatment and medication adherence.
• Participate in the development of discharge plans for each hospitalization and assure timely access to follow-up care post-discharge.
• Educate patients and their families on independent living skills and set attainable goals.

Requirements

• Bachelor's degree in a health-related field such as counseling, psychology, social work, or other behavioral sciences.
• At least two years of experience in a behavioral health position (preferred).
• Bilingual in Spanish.
