About the position

The Case Manager is responsible for managing the case files of youth in a residential facility, ensuring compliance with service plans and assessments. This role requires flexibility in scheduling, as it involves extended hours, weekends, and on-call duties. The Case Manager conducts intake interviews, coordinates with legal representatives, and documents all interactions with youth and their families, aiming to facilitate reunification and provide necessary support services.

Responsibilities

• Conduct initial intake interviews of youth and gather necessary information.
• Verify information received from minors by interviewing family members and sponsors.
• Determine options available for youth and proceed with documentation for reunification.
• Coordinate with local pro bono attorneys for legal presentations to youth.
• Ensure timely completion of assessments and service plans as per requirements.
• Document all actions and contacts with youth and stakeholders in progress notes.
• Submit reunification packets for review and ensure timely release of youth to designated representatives.
• Provide weekly updates to youth and telephonic updates to family members or sponsors.
• Facilitate incoming calls between minors and approved caregivers.
• Coordinate weekly treatment team meetings with shelter representatives.
• Maintain physical and web-based client files and ensure proper supervision of youth.

Requirements

• Bachelor's degree in Social Work, Psychology, Human Services, Counseling, or related field.
• 1 year of experience preferred in youth services or working with adolescents.
• Computer literate with knowledge of Microsoft Windows (Word, Excel).
• Bilingual in English and Spanish is required.
• Must be at least 21 years of age at the time of hire.
• Valid Driver's License.
