About the position

The Case Manager is responsible for managing the case files of youth in a residential facility, ensuring compliance with service plans and assessments. This role requires flexibility, organization, and the ability to work extended hours, including weekends and on-call shifts. The Case Manager conducts intake interviews, coordinates with legal representatives, and maintains documentation in both physical and web-based formats.

Responsibilities

• Conduct initial intake interviews of youth and gather necessary information.
• Verify information received from minors by interviewing family members and sponsors.
• Determine options available for youth and proceed with documentation for reunification.
• Coordinate with local pro bono attorneys for legal presentations to youth.
• Ensure timely completion of assessments and service plans as per requirements.
• Document all actions and contacts with youth and stakeholders in progress notes.
• Complete and submit reunification packets for review.
• Provide weekly updates to youth and telephonic updates to family members.
• Facilitate incoming calls to minors with family members and caregivers.
• Coordinate weekly treatment team meetings with shelter representatives.
• Maintain physical and web-based client files.
• Drive clients to facilitate program services as needed.

Requirements

• Bachelor's degree in Social Work, Psychology, Human Services, Counseling, or related field.
• 1 year of experience preferred in youth services or working with adolescents.
• Computer literate with knowledge of Microsoft Windows (Word, Excel).
• Bilingual in English and Spanish is required.
• Cleared Tuberculosis test results and background check.

• Multilingual skills in Turkish, Arabic, Punjabi, Hindi, Bengali, or Portuguese.
