About the position

The Case Manager at the Southern Arizona AIDS Foundation provides comprehensive case management services to individuals living with HIV/AIDS and their families. This full-time position focuses on supporting clients in accessing medical care and supportive services, ensuring they receive the necessary assistance to maintain their health and housing stability.

Responsibilities

  • Provide client-centered services to support persons living with HIV/AIDS in obtaining or maintaining primary medical care and housing.
  • Conduct initial assessments of service needs using the Self-Sufficiency Matrix and Acuity Scale.
  • Complete the Vulnerability Index- Service Prioritization Decision Assistance Tool (VI-SPDAT) for clients requesting housing.
  • Develop comprehensive, individualized service plans for clients.
  • Coordinate services required to implement the service plans.
  • Monitor clients to assess the efficacy of the service plans and adapt them as necessary.
  • Advocate for clients and review utilization of services.
  • Develop goal plans in collaboration with clients to address transition needs and long-term housing stability.
  • Support clients in meeting their identified needs.
  • Participate in weekly group supervision meetings and treatment team meetings with medical providers.
  • Conduct home visits to assess and support clients, identifying unmet needs.
  • Maintain timely and accurate documentation of all client contact.
  • Comply with SAAF employment and quality assurance policies and procedures.

Requirements

  • Bachelor's degree in social services or health-related field or 2 years' work experience in a social services related field.
  • Proficiency with Microsoft Office.
  • Valid Arizona driver's license and reliable transportation.
  • Ability to obtain fingerprint clearance through the Arizona Department of Public Safety.

Nice-to-haves

  • Knowledge of HIV/AIDS issues.
  • Two years' case management experience working with people with substance abuse and/or mental health issues.
  • Knowledge of supportive housing programs and issues related to homelessness.
  • Previous experience in a team leader or supervisor role.
  • Experience working with Tucson human service providers.
  • Effective oral and written communication skills.
  • Proficiency with database systems.
  • Strong organizational skills.
  • Excellent interpersonal skills, sensitivity to cultural and personal diversity.
  • Bilingual (English/Spanish).
