About the position

ServiceNet is seeking an experienced case manager to join their Shelter and Housing Division. The role focuses on providing support and advocacy to guests, helping them transition into permanent housing and regain self-sufficiency. The case manager will conduct assessments, facilitate referrals, and contribute to a positive residential environment.

Responsibilities

  • Conduct intakes and assessments for guests as they enter the program
  • Provide support and advocacy to shelter and transitional housing recovery participants
  • Assist participants in accessing and utilizing treatment, social services, medical, vocational, and housing resources
  • Hold weekly meetings with families/individuals to review service treatment plans and set goals
  • Contribute to the creation of a positive residential milieu in collaboration with participants and shelter staff
  • Write clear and objective case management notes and progress notes

Requirements

  • High school diploma or equivalent, plus additional training equal to two years of college
  • One year of experience working with low-income individuals or those affected by mental health issues and/or addiction
  • Knowledge of surrounding area social service agencies
  • Valid Driver's license and an acceptable motor vehicle record
