About the position

The Case Manager at H. Lee Moffitt Cancer Center is responsible for coordinating care and services for selected patient populations throughout their healthcare journey. This role emphasizes effective utilization of healthcare resources, maintaining knowledge of payer information, and leading interdisciplinary teams to achieve optimal clinical and resource outcomes. The Case Manager also represents the department in meetings with senior administration and provides expert services across the organization.

Responsibilities

  • Coordinate care and services for selected patient populations across the continuum of care.
  • Promote effective utilization and monitoring of healthcare resources.
  • Maintain accurate knowledge of payer information for team collaboration.
  • Assume a leadership role with the interdisciplinary team to achieve optimal clinical and resource outcomes.
  • Represent the department in meetings with senior administration.
  • Provide enhanced expert services to all departments within the organization.
  • Demonstrate ongoing leadership within the hospital, community, and professional organizations as required.
  • Conduct discharge planning for inpatients.
  • Perform utilization review for inpatients.

Requirements

  • Bachelor of Science in Nursing (BSN).
  • Minimum of 5 years related experience in healthcare.
  • Recent case management experience.
  • Demonstrated experience in oncology nursing, utilization review, quality management, and/or case management.
  • Computer literate with knowledge of cost accounting and financial data analysis.
  • Familiarity with Medicare and Medicaid regulations, third-party reimbursement, medical record coding, and pricing of services.
