Responsibilities

• Provide information and education to help individuals understand the Medicaid waiver process.
• Create an Individual Support Plan based on the strengths, needs, goals, and desires of the individuals served.
• Help individuals access services and supports they need to meet their goals.
• Facilitate regular community-based team meetings to review the plan of care and collaborate with service providers as needed.

Requirements

• A valid driver's license, car insurance, and reliable transportation.
• A Bachelor's Degree in Human Services.
• A minimum of one year of experience working with individuals with intellectual and developmental disabilities.
• Designated home office setting with computer, internet service, and smartphone.
