About the position

The Case Manager at Avenues Recovery is responsible for advocating for clients and addressing external barriers to treatment. This role involves initiating and overseeing case management for clients, ensuring their needs are met throughout their treatment journey, and facilitating comprehensive discharge planning. The position is crucial in providing support and resources to clients as they transition out of treatment and into their next steps in recovery.

Responsibilities

• Advocate for clients, addressing and resolving any external barriers to treatment.
• Initiate case management for each client upon intake, addressing any pressing needs.
• Oversee client accounts, ensuring insurance is active throughout the duration of treatment.
• Provide ongoing case management throughout the duration of treatment.
• Arrange housing, financial aid, legal aid, vocational training, etc. as necessary for client post-discharge.
• Facilitate comprehensive discharge planning (continuing care, housing, sober support, etc.).

Requirements

• High school diploma or GED preferred.
• Minimum 1-year experience in the treatment industry preferred.
• If in recovery, a minimum one-year period of sustained sobriety is required.
• Excellent communication, multitasking, and time management skills.
• Strong problem-solving and critical thinking skills.
• Warm, empathetic, respectable personality.
