About the position

The Case Manager at PMHCC Inc. is responsible for providing recovery support services to individuals facing challenges related to substance use disorders and co-occurring mental health issues. This role emphasizes a collaborative approach, empowering clients to identify their strengths and work towards their recovery goals. The Case Manager will maintain a caseload, connect clients with community resources, and ensure comprehensive documentation of services provided.

Responsibilities

  • Provide case management services for adults diagnosed with substance use and/or co-occurring disorders.
  • Maintain a caseload of 25 cases, meeting with each participant at least once per month.
  • Achieve 100% productivity based on available service units per month.
  • Provide case management services in the community at least 85% of the time.
  • Ensure all service provision documentation is entered within 48 hours and is accurate.
  • Assist participants in identifying goals and developing plans for success.
  • Coordinate clinical interventions and participate in aftercare planning from inpatient facilities.
  • Attend mandatory trainings and participate in bi-weekly supervision with a supervisor.

Requirements

  • High School or Associate's degree with 12 semester credit hours in social science and 2 years experience in human services, including 1 year in direct client contact.
  • Bachelor's degree in sociology, social work, psychology, or related field.
